HEP C BRIDGE COUNSELOR

CareReach has a full-time Hepatitis C Bridge Counselor position available in Buncombe County. The Bridge Counselor links people infected with Hepatitis C to clinical providers for viral hepatitis care and treatment, in addition to community resources that will aid their treatment. Duties include outreach, navigation, and education for patients with Hepatitis C, as well as reporting on these activities.

Qualifications

  • High School diploma or equivalent required, 2 or 4-year degree a plus
  • One to two years of employment in a health care setting is preferred
  • Experience in direct patient outreach and education is desired
  • Knowledge of and experience with navigating the health care system is desired
  • Coursework or experience that demonstrates an ability to utilize Microsoft products and medical software is preferred
  • Must possess a valid NC driver’s license, auto insurance and dependable transportation
  • Excellent interpersonal communication skills
  • Strong organizational, analytical, and time management skills
  • Ability to work independently and as part of a team
  • Ability to type at least 35 words per minute, this will be tested

PROGRAM MANAGER

CareReach has an opening for a full-time Program Manager. The Program Manager oversees all CareReach programs and program staff.

Primary job duties include, but are not limited to, supervising program staff, improving, and implementing programs, ensuring program deliverables are successfully achieved, assisting with grant writing and reporting, furthering community initiatives and partnerships, and developing and maintaining partner relationships.

This position will be located primarily in Buncombe County with work to be performed across the greater Western North Carolina region.

Qualifications

  • Bachelor’s degree in social work, or a related field, required. Master’s degree in Social Work, Public Health, Health Administration or Public Administration preferred.
  • Prior supervisory experience strongly preferred
  • A mature understanding of diversity, equity, and inclusion in all internal and external work.
  • Previous work experience that demonstrates capacity to successfully perform the duties of the role.
  • Coordinates and implements programs that support the organization’s mission, vision, and goals. Monitors CareReach program operations for quality, efficiency, and effectiveness.
  • Demonstrate knowledge of or ability to work with and/or within targeted communities and populations.
  • Ensures program milestones and goals are met with adherence to approved budgets.
  • Ability to speak Spanish a plus.
  • Three or more years of employment in a healthcare or social service setting is preferred.
